Rachelle Lee Crowder, MD, is a primary care physician at Goshen Physicians Family Medicine Keystone. As a family physician who enjoys taking care of people of all ages, Dr. Crowder appreciates the relationships that she has with her patients. She considers it an honor to be part of their lives. Her training and experience have prepared her to care for most of their physical, emotional and spiritual needs.
Dr. Crowder finds great satisfaction in the relationships that she develops with her patients and enjoys partnering with them in their care. Angie Mattern is a board certified family nurse practitioner at Goshen Physicians Keystone. Prior to her becoming a nurse practitioner, she worked as a registered nurse in family practice, emergency and mental health. Angie’s focus is health promotion and disease prevention through evidence based practice.
Angie enjoys developing relationships with patients and their families to provide an individualized plan of care. She spends quality time with her patients to understand their concerns and identify ways to provide them with exceptional care. Angie is passionate about preventive care and therefore provides screening assessments and education to all of her patients on a regular basis. She believes in a holistic approach to health care that addresses emotional, physical, social and spiritual care. The goal of her practice is to provide integrated patient care in the family practice setting, to empower the individual to achieve wellness.

Colleen Shay is a board certified Family Nurse Practitioner with Goshen Physicians Family Medicine Keystone. She provides care for patients of all ages, from newborns to seniors. In addition to wellness exams and routine preventive care, Colleen treats patients with common illnesses such as colds and flu. She also helps patients manage chronic conditions including diabetes, heart disease and high blood pressure.
Strong patient and provider relationships are the foundation of Colleen’s approach to care. She also works closely with doctors and other providers to ensure patients receive complete care for their medical and emotional needs.
