Why document your healthcare wishes

Advance care planning helps you identify how you want to live if you face a serious illness or life-limiting injury. Your decisions express deeply personal perspectives on what makes life worth living. They reflect your values, faith and culture.
 
When you put your directives into a written document, you lift a burden from family members and caregivers. They can turn to your advance care plan for guidance to make healthcare decisions on your behalf if you are not able to communicate your preferences.

Indiana advance directives documents

Here are advance directives documents and forms for the state of Indiana that you can download from the Indiana Department of Health:
