We work with physician offices and patients to schedule Outpatient Services including:
- Radiology/Imaging
- Lab
- Cardiopulmonary Services
- Sleep Center
- Clinical Dietitians
Patients can request lab appointments via the Patient Health Portal.
When scheduling an appointment:
- We will verify patient demographics including name, date of birth, address, phone number and insurance including subscriber, policy number and group number.
- All orders/requisitions are to be faxed to (574) 364-2410 or sent electronically through our system PRIOR to calling for an appointment.
- Additionally, we also require preauthorization for any test requiring one to be faxed to (574) 364-2410 or electronically sent PRIOR to calling for an appointment.

All Appointments
- Patient preference for appointment (day of week or time of day)
- Patient's primary language - including if an interpreter will be needed for appointment
- Current patient demographic and insurance information
- Name of ordering provider
- ICD-10 Code, need both the code and the verbiage
Imaging Appointments
- Patient's weight and height
- Is patient allergic to iodine or gadolinium?
- Does patient have renal disease?
- Date and location of any comparison films
Exposure prevention protocol (COVID positive, active TB or bug/pest infestation):
If a patient WILL NOT pass screening, we will assess with the modality affected for next steps, which may include the following:
- Depending on the situation, the patient may need to be rescheduled until the risk is reduced
- Arrange for the patient to enter through an alternate entrance and get registered over the phone prior to entering facility.

We invite our patients to engage with us electronically for the following:
- Pre-register for upcoming appointments using their smart phone or email
- Confirm appointment reminders using text or email
- Request lab appointments via the Patient Health Portal
We will contact patients prior to their date of service to:
- Verify demographics
- Verify insurance eligibility
- Provide an estimate of the service
- Provide an opportunity for the patient to make payment in advance
- Provide contact information regarding Financial Assistance Services we offer
Please note: All appointments are encouraged to be scheduled in advance, allowing us to provide efficient and safe check ins. Patient should bring a current copy of their insurance card and photo ID to their visit.

**Any patients with scheduled arrival times after 6:00 p.m. Mon-Fri will be asked to park in the Emergency Department parking lot and enter the hospital through the Emergency Room Department Entrance**
