At Goshen Health one of our priorities is to remove the barriers that prevent patients from accessing the care they need, in ways that are most beneficial to the patient. And, we recognize the need to deliver care to patients even before they get sick. Through donor support, community partnerships, and innovative thinking we’re developing new ways to meet these needs.

Through the capabilities of telehealth, combined with our nurse care coordinator team, we are now able to monitor vital indicators for patients in their homes. In 2015 a pilot program was implemented utilizing 40 telehealth units.

As we continue to see changes in healthcare we also see the needs to serve patients in new and innovative ways. Donor investments have never had more impact. Goshen Health Foundation’s role in connecting those that are inspired to give back with the possibilities we have for patient care really makes a difference.

Telehealth avoids unnecessary trips to the emergency department, and reduces hospital admissions and readmissions, enhancing patient and provider satisfaction.
