We are committed to making our community the healthiest place to live, work and raise a family. That's why we regularly identify health needs in our area and focus our resources on programs that improve access to outstanding care and services.
Community Health Needs Assessment
Every three years, we take a close look at our communities’ healthcare needs. We use focus groups, surveys, individual interviews and data to give us deeper insight into what matters most to those we serve. As we listen, learn and collaborate with community partners, we recognize ways we can make a difference in the lives of those around us.
Our Community Health Assessment (CHNA) report highlights our findings and action plan to broaden our outreach programs, expand partnerships and increase healthcare education to meet the needs of those who live and work here.

Community Benefit Fund
Our Community Health Needs Assessment guides us in establishing priorities for the distribution of Community Benefit Fund grants. Organizations use grant funds to create and support prevention and education programs throughout our community. The fund is administered by Goshen Health Foundation.
