Advance Care Planning is for Adults of All Ages

If tragedy or illness leaves you unable to speak for yourself, advance care planning can help you to receive the medical care that you want and ensure that your loved ones know your wishes. Written documents give clear instructions about the kind of care you want in case of serious health crisis or at end of life. By writing down your wishes, you can help your loved ones and doctors understand and follow your choices. That’s important if you are unable to make decisions or communicate for yourself.

In this session, you will meet individually with a Goshen Health Social Worker or Chaplain who will help you:

  • Understand the role of an Advance Care Plan
  • Complete a Health Care Representative document

It is recommended to have a family member or close friend that will be involved with your health care decisions attend the session.
