Internship Placement Process
Internship approval is a collaborative effort between the student, their college or university, and the department where the internship is requested. Final approval is granted only after all required documentation has been submitted and reviewed by:
- The Colleague Education Department
- The host department
Your application is considered complete once all required materials have been reviewed, signed, and submitted.
Requirements:
Please upload proof of the following with your Internship Application Form:
- Required Immunizations
- MMR (Measles, Mumps, Rubella) or Rubella titer
- DTaP
- Varicella (Chicken Pox)
- Hepatitis B (highly recommended, but not required)
- Negative TB Test: Skin test, QuantiFERON Gold, or recent chest x-ray
- Flu Vaccination: Required for onsite participation between October 1 – March 31
- Criminal Background Check: Submit documentation from your school or pay for and complete a search here https://www.in.gov/ai/appfiles/isp-lch/

Department-Specific Notes
- Surgical Services: Internship opportunities in surgical areas are limited and subject to specific eligibility criteria.
- Rehabilitation Department: Students interested in Speech Therapy, Occupational Therapy, or Physical Therapy may have additional requirements. These will be communicated as applicable.
Next Steps
- Once we receive your completed application and documentation, we will forward your materials to the requested department for review.
- Please note: Submission of paperwork does not guarantee placement. Final approval depends on departmental availability and alignment with your academic goals.
- If selected, you will need to submit a Badge/Parking Form in person to Goshen Health Human Resources.
- We will confirm that there is a current affiliation agreement between your college or university and Goshen Health
