Using data-driven, evidence-based practices, Community Engagement collaborates with partners to create and implement innovative education and wellness programs that impact the health needs of the community. Our programs include:

General health and wellness programs - We offer education on a variety of general health and wellness topics to schools and general community groups. These programs are available at no charge and are targeted toward specific needs.

Special events - We organize events to impact the community’s health needs.

Sprouts Kids Club - Between May and October, kids can participate in Sprouts Kids Club, a free program held on Tuesdays from 3-7 p.m. at the Goshen Farmers Market.

Summer Health Lunch Bunch - Goshen Health partners with local schools to offer fun, interactive health education and activities at select summer feeding sites.

Please begin by calling us. The emergency physician will help expedite the triage process and ensure that your patient’s care is efficient and directed to the consultant of your choice. Be prepared to provide any pertinent medical information, along with a medication list and any known allergies.

A Level II Emergency Center, we provide experienced emergency physicians and nurses who are ready to deliver efficient, effective care to patients with any type of medical emergency.

All physicians staffing our Emergency Department are board-certified or board-eligible in Emergency Medicine, something not commonly found in communities this size. Most of our physicians have been with the department for over 10 years, reflecting their commitment to provide quality emergency medicine to our community.

Follow-Up Care

We strongly encourage patients to seek follow-up care with their physician after a medical emergency. To help you provide the most complete care, we provide referring physicians with a copy of the face sheet and the Emergency Department assessment on the next business day after a patient’s visit. The referring physician’s name is added to Goshen Hospital’s Meditech account so physicians with access to electronic medical records (EMR) can view their patient’s medical records.

Home Care & Hospice have been consolidated into one department, but both provide individualized care to meet the needs of each patient and family.

Our access to patient records enables us to complete all necessary forms and orders. Then we just need the physician’s approval to proceed. We’ll contact the patient to implement the treatment plan.

We provide professional healthcare services in the privacy and comfort of a patient’s own home, relying on advanced technology and treatment methods. Studies have shown that being in familiar surroundings, as well as being closer to family, help patients recuperate sooner.

We offer individualized care based upon each patient’s specific needs. All of our providers are specially trained and registered, licensed or certified in their area of expertise.

Availability

Home care services are available 24 hours a day, seven days a week. The program accepts Medicare, Medicaid and most insurance plans.

  Hospice Admission Criteria and Guidelines

Hospice care emphasizes the quality of life as being as important as the length of life. It employs a special team approach to caring for people with an incurable, progressive illness. Hospice care may take place in the patient’s or a family member’s home, recognizing that a nursing facility may where the patient lives. Hospice care:

• Emphasizes symptom management of pain and physical discomfort
• Provides for emotional and spiritual needs of patients and families
• Supports family members in caring for the patient

Patient eligibility

Persons are eligible for hospice if they meet the following criteria:

• They no longer desire curative treatment
• The physician certifies a prognosis of six months or less
• The patient has a caregiver or plans for a caregiver

Payment

Hospice is paid for by:

• Medicare hospice benefits
• Medicaid hospice benefits
• Most insurance companies
• Individuals private pay

Note that Goshen Hospice Services are provided regardless of the ability to pay.

Trained technicians who care

Our licensed respiratory therapists and specially trained home medical technicians come to your patient’s home and set up equipment. They will also instruct your patient or a family member on proper use. Follow-up home visits are made when needed and we maintain our rental equipment while in use. Emergency assistance is available 24 hours a day. We offer an ATP certified technician, trained specifically to address seating and mobility needs.

Insurance claims

We offer assistance with Medicare and/or private insurance. We complete the required claim forms and handle all necessary correspondence. Medicare assignment is generally accepted.

There’s no need to research if a patient's insurance is in network as we are in network with most insurance companies.

Goshen Hospital is proud of our Hospitalist Program. We partner with Apogee Physicians, the nation’s leading hospitalist group, to allow for greater physician flexibility, while ensuring our patients receive the highest quality inpatient care.

Our hospitalist team is made up of internal medicine physicians, nurse practitioners and patient information coordinators.

Objectives

• To provide inpatient care for unassigned patients via the emergency department
• Unassigned patients are assigned to a PCP in their geographical area for follow-up care upon admission. This ensures complete and accurate communication with the assigned PCP, resulting in a smooth and seamless transition from inpatient care to follow-up care.
• To provide inpatient care for medical staff members (outside of Goshen) who do not currently admit patients directly.
• Co-management of inpatient care for the Goshen Physicians specialists.
• Transition to coverage for inpatient care for PCPs who choose to utilize this service.

Patient information coordinators (PICs)

The role of the patient information coordinators (PICs) is to communicate with both the patient as well as the PCP’s office to ensure a seamless transition from inpatient to follow-up care.

Responsibilities

• Contacts referring offices to notify physician of a patient’s admittance and/or obtain necessary information on admitted patients.
• Communicates with the referring physicians regarding admitted patients.
• Ensures the discharge summary is sent to the referring physician’s office within 48 hours of discharge.
• Follows up with patients after discharge to confirm the patient’s appointment with their PCP.

The PICs do not provide clinical care to patients.

Depending upon the type of test, imaging services may be performed at Goshen Hospital, Goshen Retreat Women's Health Center or the Outpatient Imaging Center on the retreat campus. Referral Form

A patient must have a physician’s referral to make an appointment. First, schedule an appointment, then please fax the referral form.

Our registered dietitian nutritionists provide counseling services to prevent or manage disease while assisting patients in setting realistic and achievable nutrition goals. Our dietitians are degreed professionals registered by the American Dietetic Association and certified by the state of Indiana. Making healthy nutrition and lifestyle changes can greatly enhance your patients’ health while complementing any treatment plan you prescribe.

The Alice A and Rex Martin Infusion Center is located at Goshen Center for Cancer Care (connected to Goshen Hospital). We provide a wide variety of infusion and injection services in a relaxed setting where patients (and their families) feel comfortable and genuinely cared for by our friendly and personable staff.

Our infusion rooms include:

• Comfortable recliner seating designed for patients receiving infusions
• Televisions so they can watch regular TV, play video games or watch relaxation videos of nature scenes
• Meals, nutritious snacks and beverages are available

• CADD pump
• Implanted port flushes and port blood draws
• Intramuscular, subcutaneous, IV push and IV infusions for antibiotics, diuretics and hydration
• Iron infusions (Venofer)

• Osteoporosis medications (Reclast infusions, Boniva IV, or Prolia SQ)
• PICC line care, declotting and removal
• Rabies series (after initial treatment of wound in the office or ED)

Instructions for scheduling patients

Please follow these steps:

1. Fill out the updated DTS order.
2. The ICD-10 codes can be obtained from the provider managing the care.
3. A provider with admitting privileges must be included on the order.
4. Email the order document. Please include ‘confidential’ in the subject line to ensure the email is secure.
5. Following our current process, the precertification number must be provided by the referring office prior to therapy.
6. In the case of a blood transfusion, biologic or chemotherapy order, verification of a signed informed consent must be presented with the precertification. No patient with an order for blood transfusion, biologic, or chemotherapy infusion will be scheduled or treated without informed consent verification.
7. If same day add on, please email the order and call (574) 364-2963 for approval and to confirm arrival time.
8. The DTS order will be reviewed. Once accepted, the infusion center scheduling department will call the ordering provider’s office with an appointment date and time. For the initial infusion center visit, the ordering provider’s office is responsible for informing the patient of the scheduled date and time. Subsequent visits (if needed) will be communicated with the patient by the infusion center scheduling department.
9. If an incomplete DTS order is received, the infusion center scheduling department will notify the provider’s office for completion before acceptance of the order or scheduling of the patient.

As a reminder, all patients must have scheduled appointments. Walk-in services are not accepted.

Following an injury, illness, surgery or trauma, Goshen Rehabilitation can help your patients (adults and children) achieve their highest potential. Outpatient Medicare B Cap

Medicare Part B places a payment cap on outpatient rehabilitation services for all providers except hospital outpatient departments. That means there are no limitations on your patients’ quality or quantity of care for outpatient services through Goshen Rehabilitation.

Outpatient services through other providers such as skilled nursing facilities, rehabilitation agencies, clinics and home health agencies are annually capped per person at less than $1,800 for occupational therapy and $1,800 for physical and speech therapy combined. Since Goshen Rehabilitation falls outside of the Medicare B cap, your patients can take advantage of their rehabilitative care as prescribed – meaning a faster road to recovery.
