- a valid email address on file with Goshen Health
- your medical record number (available from your provider) or social security number

Portal update

We’ve updated our patient portal to improve communication and information sharing. Medical records for all Goshen Health System entities will now be integrated so your providers will have access to them. The Goshen Physicians InteliChart Patient Portal is still accessible for records before 5/31/23.
